To save you time and money, we ask that you print and fill out the following forms at home. This will streamline the process of getting you in to see one of our qualified counselors, without having to waste your valuable time filling out paperwork in the lobby.
If you are unable to print out these forms from home, don't fret! We have copies of all of the forms available to you at any of the Samaritan Counseling Center offices. We just ask that you come in about 45 minutes ahead of your scheduled appointment to have time to fill them out.
If you do not have access to a computer/printer, we can also mail them to you if you so choose. Just give us a call at (269)926-6199 and ask to have the "New Client Forms" packet mailed to your specific address.
